<description>Do this step by step, i will explain things along the way.

1st. Do Ctrl+Alt+Del

2. This should open up TaskManager

3. Go to Start new Process...

4. Type Explorer.exe
Explorer.exe controls file viewing and graphical effects on the desktop. Some things it does are:
Shows windows
Opens folders
shows and operates the start menu and app tray.

5. Your desktop should come back up. If not, explorer.exe may have been damaged, and is unrepairable, then you must reformat your hard drive.

6. When your icons and start bar come back up, look at your processes, if you know which ones are suppose to operate, try to find and remove the process using search, or use IE8, FireFox, or another browser to download a AV program, and delete the Trojan Horse.

Some programs I recommend are:

Free:
AVG Free
Malware bytes

Trials:
Nod32 AV Trial
Kalpersky Trial
Panda AV Trial

Retail Products:
Norton 360
Nod32 AV
AVG 8
